Informed Consent & Participation Agreement
By enrolling in or participating in any program, service, course, or offering provided by DRIVA, DRIVA Life, DRIVA Institute, or DRIVA Compass, you acknowledge and agree to the following:
Nature of Services
I understand that DRIVA provides educational and wellness-oriented support focused on understanding and supporting the drivers of health, including lifestyle, behavior, stress, recovery, meaning, and inner coherence.
I understand that DRIVA services are not medical care.
No Medical Advice or Treatment
I acknowledge that:
- DRIVA does not diagnose medical conditions
- DRIVA does not prescribe medications
- DRIVA does not provide medical treatment
- DRIVA does not offer emergency or urgent care
Any information provided through DRIVA is educational and supportive in nature and is not a substitute for professional medical advice, diagnosis, or treatment.
No Doctor–Patient Relationship
I understand that participation in DRIVA programs does not create a doctor–patient relationship between myself and DRIVA, its founder, or any associated contributors.
All medical decisions remain my responsibility and the responsibility of my licensed healthcare providers.
Role of Medical Care
I understand that:
- Medical care may be necessary for diagnosis, treatment, or emergencies
- DRIVA encourages appropriate medical evaluation and treatment when needed
- DRIVA services are intended to complement, not replace, medical care
I agree to consult my physician or another qualified healthcare provider regarding any medical concerns.
Personal Responsibility
I acknowledge that:
- My participation is voluntary
I am responsible for my own health decisions - I will use my judgment in applying any educational or wellness guidance
- I will seek medical attention if symptoms worsen, change, or become concerning
No Guarantee of Results
I understand that:
- Health outcomes vary between individuals
- DRIVA does not guarantee specific results, improvements, or outcomes
- Participation does not ensure healing, recovery, or longevity
Health is influenced by many factors beyond any single program or approach.
Medical Emergencies
I understand that DRIVA is not appropriate for medical emergencies.
If I experience severe or concerning symptoms—including chest pain, shortness of breath, severe pain, sudden weakness, or changes in consciousness—I agree to seek immediate medical attention or contact emergency services.
Spiritual Language Clarification
I understand that references to spirit within DRIVA content refer to:
- Meaning
- Purpose
- Identity
- Inner coherence
and do not refer to religion or religious belief.
